IMPORTANT INFORMATION ABOUT YOUR PERSONAL TRAVEL INSURANCE
This insurance is arranged by Citybond Suretravel, a division of Citybond Holdings plc. It is underwritten by Mondial Assistance Europe N.V. and administered in the United Kingdom by Mondial Assistance (UK) Limited. Mondial Assistance (UK) Limited and Citybond Suretravel are authorised and regulated by the Financial Services Authority.
Your policy contains restrictions regarding existing medical conditions of the people travelling and of other people upon whose health the trip may depend.
The Citalia Holidays Travel Insurance operates on the basis that all insured persons are not travelling against medical advice or with the intention of obtaining medical treatment abroad.
You will not be covered for any directly or indirectly related claims arising from the following if at the time of taking out this insurance, you:
- have been prescribed any medication, received any treatment or had a consultation with a doctor or hospital specialist in the past 2 years;
- have EVER been diagnosed with or treated for any cardiovascular problems (e.g. heart attack, angina, chest pain, palpitations, any other heart condition, hypertension (raised blood pressure), blood clots, raised cholesterol) or any cerebrovascular problems (e.g. stroke, transient ischaemic attack, brain haemorrhage), at any time prior to cover;
- are awaiting treatment or the results of any tests or investigations.
Unless
Your medical condition is listed under the accepted conditions (see policy wording) or all relevant condition(s) are declared to and accepted by us in writing.
To declare an existing medical condition or a change in your state of health or prescribed medication, you should contact Citybond Suretravel’s Medical Screening Department, Mondial HealthScreen, at www.mondialhealthscreen.co.uk, or telephone them on 0845 618 0344 and quote ‘Citalia Holidays’ (Mon. to Fri. 9.30am to 5.00pm, Sat 9.30am to 12.30pm). Please note that calls to Mondial HealthScreen may be monitored or recorded for audit or training purposes.
The policy wording gives you full details of what is and what is not covered and what to do if you need to claim. Your policy does not cover all possible events and expenses. It is very important that you read the whole of this policy before you travel and make sure the cover is suitable for you.
